ABSTRACT

High-risk drugs have strong efficacy and can cause remarkable patient harm or drug dependence, leading to serious consequences, when they are used in error. Management and use of high-risk drugs is an important part of clinical drug safety management. Western medicine pharmacy is the core department and an important link of drug management and use. We reviewed the literature on the management and use of clinical high-risk drugs at home and abroad, investigated the current main problems and influential factors, analyzed the main intervention measures on the management and use of high-risk drugs, so as to increase physician's and pharmacy's attention to high-risk drugs, standardize scientific use, and avoid medical accidents due to improper management and use of high-risk drugs. All of these will help us to put forward corresponding management strategies and further standardize the management and use of high-risk drugs.

ABSTRACT

Objective To explore the main causes of the overuse of proton pump inhibitor (PPIs) in domestic hospitalized patients. Methods The electronic medical records of patients in 20 general hospitals from January 1, 2015 to March 31, 2018 were analyzed. The distribution of indications of PPIs, the distribution of different uses of PPIs, the most often diseases of patients using the off-label medications, surgeries and combined medication, etc. were analyzed. Results The number one indication of PPIs was the prevention of NSAIDs-related ulcer, which accounted for 40.9% of the patients and 34.2% of them was off-label users. Among the off-label patients, lansoprazole was the most highly used, which accounted for 48.1%. The most often diseases, surgeries and combined medications of the patients using PPIs were the diseases of esophagus, stomach and duodenum, gallbladder and biliary surgery and mineral supplement. Conclusion The prevention of drug-induced ulcers and the off-label use is the main reasons for the huge amount of PPIs usage. The application of lansoprazole is not standardized; The main causes of the off-label PPIs usage are gastrointestinal discomfort, fasting, surgery and glucocorticoids.

ABSTRACT

The hospital introduced a multi-department synergy in management of the rational use of carbapenems. Specifically, the medical affairs department conducts training and appraisal of doctors along with a monthly checkup of medical records. The pharmaceutical affairs division conducts prior prescriptions checkup and follow-up comment. The clinical microbiology laboratory and the hospital-acquired infection management department monitors and releases such infection and bacterial resistance information of the whole hospital in real time. The results showed increased prescriptions of imipenem and cilastatin sodium, and decreased prescriptions of biapenem for injection. Drug resistance analysis showed that carbapenem resistant strains increased by 28%,but the total number of patients reduced by 10% and total number of patients with multidrug resistance remained unchanged. It is proposed to further antimicrobial stewardship in the hospital to achieve rational drug use and curb bacterial resistance.

ABSTRACT

Pain management is the monitoring of chronic pain patients under long-term treatment with controlled analgesic drugs via random or regular lab tests. Plenty of clinical data showed that pain management could not only improve the patient′s compliance to prescribed drugs and result in better outcome, but also greatly reduce the possibility of controlled analgesic drugs into the underground market illegally. Drugs that are usually internationally recommended in pain management include opiates, opioids, sedative drugs, stimulant, hallucinogens etc. Considering drug metabolism rates difference among individuals and ease of sampling, random urine is usually the first choice of specimen for drug testing in pain management. In terms of methodology,LC-MS/MS has become a"gold standard"approach due to its high sensitivity, high specificity and wide usability.

ABSTRACT

Objective:To explore the feasibility of pharmacy trusteeship in Shanghai medical institutions to provide useful informa-tion for the reform of medicine without added profit and pharmacy transition to clinical pharmacy. Methods: On-site investigation of seven hospitals and one famous drug retail company in Shanghai was performed combined with the communication with relative person-nel and non-random sampling questionnaire survey on the staff of pharmacy. Statwing software was used in the statistical analysis of the data. Results: A total of 93 valid questionnaires in the study were taken back. Although one hospital has achieved great progress in pharmacy trusteeship, most of respondents had negative attitudes and the common thought was that pharmacy trusteeship did more harm than good to the development of clinical pharmacy at present. Conclusion: As a consequence of drug zero interest policy and clinical pharmacy development, pharmacy trusteeship should be explored and attempted. The duties and responsibilities of all parts in pharma-cy trusteeship should be definite in law in order to standardize drug retail market and protect the interests of patient.

ABSTRACT

OBJECTIVE:To seek an effective starting point for the improvement of pharmacy administration efficiency(PAE)in China.METHODS:Level of PAE in China, the existing problems and their root causes were analyzed using a systematic methodology.RESULTS&CONCLUSIONS:Level of PAE in China is far from perfect, which may be attributed to the narrowly defined boundary of pharmacy administrating system, the lack of systematic understanding and command of relevant factors and the unscientific,non-integrated objective of pharmacy administration. It is advisable to enlarge system boundary, adjust the objective of pharmacy administration, and adopt a systematic point of view and methodology in the practice of pharmacy administration.
